Your First Appointment (What to Bring)

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

Patient's insurance card and referral if required by your insurer.
List of current prescriptions and/or over-the-counter medications, including dose and frequency.
Information about patient's medical and surgical history.
Old records: Please bring any medical records with you from previous doctor visits. This is especially important for records related to your allergies or asthma or whatever it is you are seeing Dr. Dobozin for. This would include consultation letters, written reports for CT scans, MRI's, X-rays, blood tests, and relevant doctor notes.
A list of questions you might have for Dr. Dobozin.
